ORIGINAL TASK DESCRIPTION:
A Unitary PlusMinus doesn't get through the xlst step. Here's example code:

<m:mo>&PlusMinus;</m:mo>
<m:ci>Q</m:ci>
</m:apply>
</m:math>

The .fo file just drops the PlusMinus completely.
